Reconditioning Rehabilitation Day Program
About the program:
The Southport Private Hospital Reconditioning Day Program specialises in the rehabilitation of patients who have suffered an acute episode of illness or undergone a surgical procedure with subsequent deterioration in functional status. Under guidance of a Rehabilitation Consultant and an interdisciplinary team, the Reconditioning Day Program offers the patient treatment aimed at maximising their function and achieve their goals.
Who is the program for?
Patients who have had deterioration in functional status following an acute episode of illness or undergone a surgical procedure including amputation. The Program is suitable for persons who have established reconditioning rehabilitation needs but do not need the support of inpatient medical or nursing care.
Program content:
The program aims to increase mobility and fitness to ensure the optimum level of independence. Your program may include the following services
- Physiotherapy
- Hydrotherapy
- Occupational Therapist
- Dietetics
The Physiotherapists will implement a program suited to the patient’s individual needs and goals. The Program is usually based on an exercise regime at a pace suited to the patient and aims to :
- Restore mobility and fitness
- Restore balance and strength
- Regaining optimum levels of independence
- Prescription of ongoing exercise program
- Assessment of mobility aids and equipment
An Occupational Therapist also plays a vital role in helping the patient maximise functional independence in all aspects of their daily life. All patients are reviewed by the Occupational Therapist and the following areas are assessed:
- Self-care
- Functional movement
- Home activities
- Aid prescription e.g. stocking guards, easi-reacher
- Home safety assessment
- Carer’s needs, community service requirements
- Cognitive assessment
Patients can be seen by other Allied Health members including Dietitian and Social worker on a needs basis.
When is it held:
Day rehabilitation patients can attend 1-4 times per week according to their individual needs and as determined by the Rehabilitation Consultant and team members. The day rehabilitation program runs for 3 hours a session for a minimum of 4 weeks. This timeframe will be adjusted accordingly to patient overall goals.
