Your Pathway to Care:
We partner with you to create an individualised plan supporting you from admission through to recovery. Our focus is on equipping you with the necessary tools and strategies to manage your pain effectively, long after your hospital stay.
1. Getting Started
You can be referred to us by your GP, specialist, or another health provider. Once we receive your referral, our team will contact you to discuss your needs and arrange pre-admission or an assessment.
2. Planning Your Stay
Together, we’ll create a care plan tailored to you. We’ll discuss what to bring, what to expect, and who you’ll meet.
3. Your Inpatient Program
During your stay, you will take part in a structured 14-day program that may include:
- Gentle exercise and hydrotherapy
- Psychology and pain education
- Occupational therapy and goal setting
- Social worker assistance
- Art therapy, pet therapy, and relaxation sessions
- Medical reviews and medication support
Our team works together to help you feel stronger, more confident, and more in control of your pain.
4. Preparing for Discharge
Before you leave, we’ll meet with you to review your progress and plan your next steps. You’ll receive a summary for your GP and a toolkit of new strategies to use at home.
5. Ongoing Support
We’ll stay in touch with you. You may be invited to our ongoing Day Rehab Program for follow-up sessions, and we’ll help you connect with community services if needed.
